The Diffuse Large B-cell Lymphoma (DLBCL) prognosis is primarily assessed using the International Prognostic Index (IPI) and the Revised International Prognostic Index (R-IPI). These scoring systems sum points based on the presence of specific clinical risk factors:

  • Age: Patient age greater than 60 years.
  • ECOG Performance Status: ECOG Performance Status score of 2 or higher.
  • Serum LDH: Serum Lactate Dehydrogenase levels are elevated above the upper limit of normal.
  • Ann Arbor Stage: Disease stage is III or IV according to the Ann Arbor staging system.
  • Extranodal Sites: More than one extranodal site of disease involvement.

Each of these five factors, if present, contributes 1 point to the total IPI score. The resulting IPI score (0-5) is then used to stratify patients into different risk groups, influencing treatment strategies and providing an estimated prognosis.
