What is PCWP?
Pulmonary Capillary Wedge Pressure (PCWP), also known as pulmonary artery wedge pressure (PAWP), is an estimate of Left Atrial Pressure. In clinical settings, it is traditionally measured via a Swan-Ganz catheter, but can also be estimated non-invasively using echocardiography.
Clinical Significance
PCWP is a vital metric for diagnosing the cause of acute pulmonary edema and monitoring patients with congestive heart failure. It helps clinicians distinguish between cardiogenic and non-cardiogenic pulmonary conditions.
- Normal Range: Typically between 6 to 12 mmHg.
- High PCWP (>18 mmHg): Often indicates left-sided heart failure, mitral valve stenosis, or fluid overload.
- Low PCWP: May indicate dehydration or hypovolemic shock.
The Nagueh Formula
For non-invasive estimation via echocardiography, the Nagueh formula is frequently used. It utilizes the ratio of the mitral inflow velocity (E) to the early diastolic annular velocity (e').
Formula:
Nagueh Formula:
PCWP = 1.24 × (E / e') + 1.9
