The MELD-Na (Model for End-Stage Liver Disease with Sodium) score is a vital prognostic marker, derived from the following complex formulas:
1. Initial MELD Score Calculation:
MELD = 3.78 × ln(serum bilirubin) + 11.2 × ln(INR) + 9.57 × ln(serum creatinine) + 6.43
2. MELD-Na Adjustment (if applicable):
MELD-Na = MELD + 1.32 × (137 - Na) - [0.033 × MELD × (137 - Na)]
Where:
- Bilirubin (mg/dL): Serum total bilirubin. Minimum value for calculation is 1.0 mg/dL.
- INR (International Normalized Ratio): Prothrombin time. Minimum value for calculation is 1.0.
- Creatinine (mg/dL): Serum creatinine. Minimum value for calculation is 1.0 mg/dL. Maximum value for calculation is 4.0 mg/dL. If the patient has been on dialysis (at least twice in the past 7 days), creatinine is automatically set to 4.0 mg/dL.
- Na (Sodium) (mEq/L): Serum sodium level. Values used in the MELD-Na adjustment are capped between 125 mEq/L and 137 mEq/L.
