The MELD-Na score is calculated in two steps:

Step 1: Calculate the MELD Score

MELD = 3.78 × ln(Bilirubin) + 11.2 × ln(INR) + 9.57 × ln(Creatinine) + 6.43

Step 2: Adjust for Sodium (MELD-Na)

MELDNa = MELD - [0.025 × MELD × (140 - Sodium)] + 140

Where:

  • Bilirubin: Total bilirubin (mg/dL). Values less than 1.0 are treated as 1.0.
  • INR: International Normalized Ratio. Values less than 1.0 are treated as 1.0.
  • Creatinine: Serum creatinine (mg/dL). Values less than 1.0 are treated as 1.0, and values greater than 4.0 are treated as 4.0. If the patient has received two or more dialysis treatments within the last 7 days, creatinine is set to 4.0.
  • Sodium: Serum Sodium (mEq/L). Values less than 120 are treated as 120, and values greater than 140 are treated as 140.
  • ln: Natural logarithm.
