The Braden Score is derived by summing the scores from six distinct subscales:

Braden Score = Sensory Perception + Moisture + Activity + Mobility + Nutrition + Friction & Shear

Each subscale is rated from 1 to 4 (except Friction & Shear, which is 1-3), with a lower score indicating greater impairment and higher risk:

  • Sensory Perception: Ability to respond meaningfully to pressure-related discomfort. (Scale: 1-4)
  • Moisture: Degree to which skin is exposed to moisture. (Scale: 1-4)
  • Activity: Level of physical activity. (Scale: 1-4)
  • Mobility: Ability to change and control body position. (Scale: 1-4)
  • Nutrition: Usual food intake pattern. (Scale: 1-4)
  • Friction & Shear: Risk related to friction and shear forces on the skin. (Scale: 1-3)

A lower total Braden Score signifies a higher risk of developing a pressure injury.
